The waiver approval included many changes one of them being the Long-Term Care Ombudsman office may now submit a letter on behalf of a member to their Care Coordination Agency CCA requesting expedited release from the ALW Waitlist due to a need for imminent services.
If you are experiencing a forced discharge before the state's required minimum day hospital admission date, we encourage you to request to speak to an APS or Ombudsman caseworker to submit an Imminent Services Prioritization letter to our agency at once to ensure placement priority is received on our waitlist. If you have a client who qualifies for Imminent Services Prioritization as documented by the Long-Term Care Ombudsman office please submit their waitlist request form along with supporting documentation to info allhoursadultcare.
Return a letter to our office on county letterhead to be placed on the APS waitlist. A ll applications for institutional transfers must be submitted to the waitlist.
Our office will be in contact with you when your referral has reached the top of the waitlist and is ready for an RN assessment. If the social worker will not be the point of contact after submission, an additional point of contact is required. This can be done by purchasing additional insurance, paying a higher fee to the ALW facility, or working with a Medi-Cal helper to clear the Share of Cost.
Care coordination services for the waiver program are free of charge to the applicant or advocate assisting with the application. Time until assessment: Approximately months. Voluntary Discharge: If you voluntarily discharge from your facility before completing a 60 day admission, you will be placed on the 18 month minimum community waitlist. Involuntary or Premature Discharge: If you feel your discharge planning is involuntary or premature, immediately request a visit from the Ombudsman at the facility and ask for an Imminent Services Prioritization Letter to the assisted living waiver program for a shorter wait time.
