The use of new risk predictors for CHD such as the high-sensitivity C-reactive Protein hs-CRP in addition to established risk factors could improve prediction of CHD. As a consequence of the altered risk assessment, modified preventive actions could reduce the number of cardiac death and non-fatal myocardial infarction. Does the additional information gained through the measurement of hs-CRP in asymptomatic patients lead to a clinically relevant improvement in risk prediction as compared to risk prediction based on traditional risk factors and is this cost-effective?

Kurzfassung: Inflammation spielt eine Schlüsselrolle in der Pathogenese der Atherosklerose. Das Risiko der Plaqueruptur ist in erster Linie von der Aktivität des entzündlichen Prozesses in der atherosklerotischen Plaque abhängig. Erhöhte Konzentrationen inflam- matorischer Substanzen, wie P-Selektin, Interleukin-6, tumor necrosis factor-alpha, soluble intercellular adhe- sion molecule-1 und C-reaktives Protein CRP , ein we- sentlicher Vertreter der Akute-Phase-Proteine, werden mit der Atherogenese in Verbindung gebracht.
Sowohl bei Patienten mit akuten Koronarsyndromen als auch bei Patienten mit stabiler koronarer Herzkrankheit wur- den im Vergleich zu gesunden Kontrollkollektiven er- höhte Konzentrationen an C-reaktivem Protein festge- stellt.
